Pregnancy में कौन-से Tests और Check-ups कब होते हैं?

Antenatal care केवल ultrasound करवाने का नाम नहीं है।

Regular antenatal contacts का उद्देश्य है:

• मां की health monitor करना
• baby की growth और wellbeing assess करना
• complications जल्दी पहचानना
• preventive care देना
• delivery की planning करना
• pregnancy-related questions और concerns discuss करना

WHO uncomplicated pregnancy में कम से कम 8 antenatal contacts recommend करता है, हालांकि individual pregnancy में visits ज्यादा हो सकती हैं।

पहला Antenatal Visit कब?

Pregnancy confirm होने के बाद early booking useful होती है।

Ideally first trimester में initial assessment हो जाना चाहिए।

इस visit में commonly discuss किया जाता है:

• last menstrual period और expected due date
• previous pregnancies
• miscarriages/caesarean history
• medical illnesses
• current medicines
• allergies
• family history
• blood pressure
• weight/BMI
• nutrition और supplements

शुरुआती Blood Tests

Clinical context के अनुसार commonly include हो सकते हैं:

• haemoglobin/CBC
• blood group और Rh type
• urine testing
• blood sugar assessment जहाँ appropriate
• HIV
• hepatitis B
• syphilis screening
• अन्य tests based on history/local protocol

Thyroid tests हर pregnancy में same तरीके से जरूरी नहीं होते; clinical risk और local practice के अनुसार decision लिया जाता है।

Dating Ultrasound

Early ultrasound pregnancy dating और viability assessment में useful हो सकती है।

यदि menstrual dates uncertain हों तो dating particularly helpful होती है।

11–14 Weeks के आसपास

Patient-specific screening options discuss किए जा सकते हैं।

इनमें:

• first-trimester ultrasound
• nuchal translucency
• chromosomal-anomaly screening options

clinical context, availability और patient preference के अनुसार शामिल हो सकते हैं।

हर patient में exact screening pathway समान नहीं होता।

18–22 Weeks: Detailed Anomaly Scan

Second trimester में detailed fetal anatomical assessment commonly किया जाता है।

इसका उद्देश्य fetal anatomy को systematically evaluate करना है।

Important: Normal anomaly scan भविष्य की हर possible fetal or pregnancy problem की guarantee नहीं है।

कोई test 100% abnormalities detect नहीं कर सकता।

24–28 Weeks: Gestational Diabetes Screening

Gestational diabetes screening commonly 24–28 weeks के आसपास होती है।

India में testing protocol local guideline/practice के अनुसार differ कर सकता है।

Blood Pressure हर Visit में क्यों?

Pregnancy-related high BP कभी-कभी symptoms के बिना भी हो सकता है।

इसलिए BP monitoring antenatal care का essential component है।

Baby Growth कैसे Monitor होती है?

Pregnancy के later months में:

• maternal abdominal examination
• fundal height
• fetal movements
• fetal heart assessment
• ultrasound जहाँ indicated हो

का उपयोग हो सकता है।

क्या हर Visit में Ultrasound जरूरी है?

नहीं।

यह common misconception है।

Uncomplicated pregnancy में routine frequent ultrasound automatically better care नहीं है।

Additional scans की जरूरत हो सकती है यदि:

• fetal growth concern
• maternal hypertension
• diabetes
• multiple pregnancy
• placental concern
• previous fetal growth problem
• bleeding
• specific maternal/fetal indication

High-Risk Pregnancy में Schedule अलग क्यों?

यदि pregnancy में:

• hypertension
• diabetes
• twins
• significant anaemia
• thyroid or medical disease
• previous serious obstetric complication
• fetal growth problem

हो तो visits और investigations अधिक frequent हो सकते हैं।

Internet Checklist से Tests करवाना क्यों उचित नहीं?

Tests का उद्देश्य “maximum tests” करवाना नहीं है।

Goal है: Right test → Right stage → Right indication

Unnecessary repeated investigations cost और anxiety दोनों बढ़ा सकती हैं।

याद रखें

Antenatal care का सबसे महत्वपूर्ण हिस्सा केवल tests नहीं है।

यह ongoing: Assessment + Prevention + Counselling + Monitoring + Birth planning है।
