Pregnancy नहीं हो रही—कब Fertility Evaluation कराएँ?

Conception में समय लगना automatically infertility नहीं है।

लेकिन सही समय पर evaluation important है, especially increasing maternal age या known reproductive problems में।

कब Evaluation?

General guidance:

Age <35

Regular unprotected intercourse के बावजूद लगभग 12 months pregnancy न होने पर evaluation consider की जाती है।

Age 35 या अधिक

लगभग 6 months के बाद evaluation useful है।

Age >40

Earlier evaluation appropriate हो सकती है।

जल्दी Evaluation कब?

12 months wait करना जरूरी नहीं यदि:

• periods absent/very irregular हों
• known PMOS with significant ovulation problems
• suspected endometriosis
• previous pelvic infection
• previous ectopic pregnancy
• ovarian surgery
• known tubal disease
• significant male-factor concern
• sexual dysfunction preventing intercourse
• chemotherapy history

Infertility किसकी Problem है?

Couple की।

Male factors fertility problems में significant contribution कर सकते हैं।

इसलिए केवल महिला के multiple tests करवाकर months later male partner evaluate करना inefficient हो सकता है।

Female Evaluation में क्या शामिल हो सकता है?

Depending on history:

• ovulation assessment
• menstrual history
• pelvic examination
• ultrasound
• ovarian reserve testing where indicated
• tubal patency evaluation

Male Evaluation

A basic semen analysis often important early test होता है।

Abnormal result के अनुसार further evaluation decide होती है।

AMH क्या बताता है?

AMH ovarian reserve estimate में useful हो सकता है।

लेकिन: AMH alone natural fertility का direct “score” नहीं है।

Low AMH = impossible pregnancy नहीं।

High AMH = guaranteed fertility नहीं।

Ovulation Apps

Apps fertile window estimate कर सकती हैं।

लेकिन cycles irregular हों तो prediction less reliable हो सकती है।

क्या रोज Intercourse जरूरी है?

Conception के लिए fertile window में regular intercourse sufficient होता है।

Highly rigid schedules unnecessary stress पैदा कर सकते हैं।

Treatment

Treatment cause और couple-specific factors पर depend करता है:

• timing/lifestyle advice
• ovulation treatment
• treatment of underlying disease
• surgery in selected cases
• IUI
• IVF/ICSI

हर couple को IVF की जरूरत नहीं होती।

Age क्यों Important है?

Female fertility age के साथ decline करती है, especially mid-to-late 30s के बाद।

इसलिए prolonged unnecessary delays avoid करना useful है।
